We are seeking a highly skilled and empathetic Licensed Practical Nurse to join our team at Crestwood Center. In this role, you will have the opportunity to make a significant contribution to the delivery of exceptional patient care and improve the quality of life for our residents.
Your key responsibilities will include evaluating patients/residents to collect data, observing condition and reporting changes in condition, contributing to nursing assessments and care planning, administering medications and performing treatments per physician orders, and supervising and monitoring patient care provided by unlicensed staff. You will work closely with our interdisciplinary team to ensure seamless coordination of care and achieve positive clinical outcomes.
