General Summary
- The Clinical Documentation Specialist/Intake Assistant works with the Intake Coordinator and the Documentation Specialist to receive and coordinate the disposition of referrals to include compiling required documents, tracking specific forms and collaborating with scheduling.
- Receives and processes referrals for agency, to include compiling required documents, acquiring following physician and tracking Face-to-Face forms.
- Collaborates with scheduling department to prioritize patient admissions.
- Collaborates with Case Managers and staff from various referral sources, such as hospitals, physician clinics and skilled nursing facilities, to coordinate admissions and transfers.
- Assists with quality review of home health OASIS time point assessments and care plans to ensure timely review and coding. Submits revisions to clinicals as indicated. Ensures clinicians complete revisions and agency processes in a timely manner. Observes for appropriate utilization of services for each home health patient episode.
- Assists with review of patient records and prepares documents when agency receives a request for additional information (ADRs) prior to submission to the MAC for review.
- Supervises telehealth patient data review as assigned.
- Graduate of an accredited school of nursing required.
- One (1) year home care experience required.
- One (1) year experience in a JC-accredited hospital preferred.
- A current Missouri RN license or Compact Licensure.
- American Heart Association BLS required.
- A valid Missouri driver's license and proof of current vehicle insurance required.
- If assigned a company vehicle, must obtain Class E license at your expense within two weeks of assignment.
- Considerable mental concentration required. Lifting up to 35 lbs. Standing, turning, walking, carrying, pushing, pulling, stooping, crouching, twisting, and reaching.
- General office environment.
