Position Overview:
Under the supervision of the Assessment Supervisor in Long Term Care Access, this role is responsible for conducting assessments using the Functional Eligibility Determination (FED) tool to determine eligibility for various care options, including Home and Community-Based Services (HCBS), nursing home admissions, personal care homes, and domiciliary care homes for individuals aged 18 and older. The position also involves limited follow-up to ensure appropriate care is provided.
Education and Experience:
- Bachelor’s Degree in Human Services, Social Work, or a related field
- Minimum of 1 year of experience in case management or assessment within social services
Key Duties and Responsibilities:
- Complete required training and certification in Pennsylvania Individualized Assessment (PIA) and Functional Eligibility Determination (FED), along with annual refresher courses to maintain certification.
- Maintain an organized electronic work schedule in the Long-Term Care Access Management System.
- Meet daily case quotas for FED assessments, conducting thorough interviews and gathering necessary information from clients, their families, physicians, and referral sources.
- Assess the suitability of Home and Community-Based Services (HCBS) or facility placement, considering the client’s preferences to determine the least restrictive, most appropriate care option.
- Complete financial screenings for applicants interested in various long-term care services, including Personal Care Homes, Domiciliary Care placement, and HCBS programs.
- Refer applicants to the appropriate services or place them on waiting lists when necessary.
- Administer the Pre-admission Screening/Annual Resident Review (PASARR) for individuals with mental health, intellectual, and developmental disabilities as required by federal law (OBRA).
- Assist clients with the Medical Assistance financial application for nursing home relocation.
- Input and maintain data in all required automated systems, ensuring timely updates.
- Accurately complete reconciliation reports and upload completed assessments to the automated filing system.
- Collaborate professionally with PCA Long-Term Care programs, providers, and referral sources.
- Participate in staff meetings, training, and problem-solving activities to contribute to the development and improvement of the program.
- Uphold and represent agency policies, procedures, and objectives to applicants, providers, and the public.
- Foster a supportive and cooperative team culture while contributing to the overall success of the team.
