What You’ll Do:
As an Assessment Worker under the guidance of the Assessment Supervisor in Long Term Care Access, you’ll:
- Use the Functional Eligibility Determination (FED) tool to assess individuals (ages 18 ) for services like:
- Home & Community-Based Services (HCBS)
- Nursing Homes
- Personal Care Homes
- Domiciliary Care
- Conduct client interviews and collaborate with families, physicians, and referral sources to gather essential information
- Recommend the least restrictive, most appropriate care options based on each client’s unique needs and preferences
- Perform financial screenings and support clients in applying for Medical Assistance and other benefits
- Accurately maintain electronic records, complete documentation, and ensure compliance with federal and state guidelines
What We’re Looking For:
- Bachelor’s Degree in Human Services, Social Work, or a related field
- At least 1 year of experience in case management or assessments within social services
- Valid driver’s license, clean driving record, and access to a fully insured vehicle
- Ability to pass drug testing and criminal history clearance
- Strong computer and documentation skills
