A discharge planner helps patients transition from the hospital to their next care setting, such as home, rehabilitation, or another facility. They work with patients, families, and medical staff to create a discharge plan that meets the patient's needs.
Responsibilities :
- Assess the patient's needs and financial resources
- Coordinate follow-up care, such as appointments and home health care
- Educate patients on medication management and post-hospital care
- Make referrals to community resources
- Ensure patients leave the hospital safely and smoothly
Strong interpersonal and organizational skills, understanding of healthcare protocols and insurance, compassionate communication and clinical knowledge.
A degree in nursing, social work, or a related healthcare field or relevant clinical experience
Salary : $32 - $38
