Clinic Nurse - RN / LPN

CAREER SUMMARY:

The Clinic Nurse's primary responsibilities will include answering patient calls to ensure excellence patient care, timely response to patient needs, seamless communication, work with chronic care management, transitional care, and remote monitoring services.

CAREER DUTIES:

• Call triage: Respond promptly to patient telephone calls, assess needs, and coordinate care with providers. Perform appropriate triage and ensure all pertinent treatment information is documented accurately in patient records.
• Clinical Nursing Intervention: Assist with tasks such as medication refills, medication, vaccine, or injection administration. Verify and maintain Vaccines for Children (VFC) program records.
• Chronic Care Management: Enroll qualifying patients into the Chronic Care Management services. Collaborate on care planning. Provide patient education, guidance, and support for chronic disease conditions.

• Transitional Care Management: Follow up with patients after discharge ensuring continuity of care. Schedule and coordinate post-discharge appointments with primary care, specialists, or other necessary services.
Medication Reconciliation: Verify patients' understanding of medication regimens. Identify and resolve any prescribed medication discrepancies.

CAREER REQUIREMENTS:

• Associate Degree or Bachelor of Science Degree in Nursing from an accredited school.
• Must hold a valid current certificate or license as a medical assistant or LPN/RN in the State of Kansas.
• Must continue CME credits in order to maintain current license.
