What are the responsibilities and job description for the Case Manager Workforce Sectoral Programs position at Phipps Neighborhoods?
Primary Function:
Under the supervision of the Director of Mental Health position, the Case Manager – Workforce Sectoral Programs drives the overall case management for the Career Network Programs (Healthcare & Building Services), direct placement (JtBO) residences vocational training, certifications and job placement services. The Case Manager will conduct a range of activities focused on helping individuals achieve desired employment outcomes including wraparound support and career navigation.
Duties and Responsibilities:
Salary Range: $50,000 to $55,000 - salary is commensurate with education, training, and experience.
- Support the design of assessments, planning, implementation, coordination and evaluation components of the case management model.
- Organize case management activities to maximize sectoral program(s) contract's goals and performance targets including attendance milestones, program deliverables
- Create and implement engagement and retention strategies for participants with varied barriers to gaining meaningful employment.
- Complete the implementation of sector-based intake process including phone screening and interviews.
- Collect, enter and maintain progress notes, and outcomes data as related to program goals and contractual obligations via Salesforce.
- Monitor and assess Workforce sectoral program performance to ensure program administration and practices result in high quality services to participants
- Work cross functionally, particularly with the Operations & Compliance, Retention Services, Employer Engagement and Outreach units.
- Perform any and all tasks deemed necessary by supervisor
Qualifications:
- Bachelor’s degree preferred, Associate's required, with at least three years of demonstrated experience working with youth and young adults in workforce, community and school-based settings.
- Strong interpersonal, public speaking and relationship-building skills with the ability to communicate with a variety of stakeholders including program participants, staff, funders and program partners.
- Strong critical thinking, analytical, and technical problem-solving skills are required.
- Ability to work independently as well as part of a team. Ability to work with people from diverse backgrounds. Self-directed and able to take initiative.
- Ability to handle matters of confidentiality with utmost tact and diplomacy.
