About the Role

Phix Health is looking for an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join us at an outpatient practice in Philadelphia, PA. This is a part time or opportunity for up to 4 days per week, depending on location.

Responsibilities :

  • Provide compassionate primary care services for a geriatric and / or disabled patients including comprehensive and focused physical examinations, health assessments, diagnoses, and treatment of acute and chronic illnesses
  • Manage chronic conditions such as diabetes, hypertension, congestive heart failure, arthritis, chronic obstructive pulmonary disease, and thyroid disorders
  • Assess need for appropriate lab tests, order and interpret results
  • Collaborate with medical specialties through appropriate referrals
  • Assist in coordination of community resources to meet needs of individuals and families, as well as appropriate follow-up.
  • Educate patients on health promotion throughout the aging process
  • Evaluates and treats patients in accordance with Phix Health’s Care Model.
  • Establishes and monitors appropriate level of care for center patients.
  • Completes all medical record documentation prior to end of shift.
  • Works with Medical Director to ensure care is operations are consistent with medical and professional standards of care.
  • Recommends potential growth opportunities for new or existing services.
  • Meets with Medical Director or designate to discuss quality of patient care, review policies, procedures, outcomes reports, and records.
  • Participates in quality management processes including encouragement of clinical support staff to assure proficiency in the delivery of clinical support activities.
  • Calls patients / family member to ensure their well-being as needed following center visits.
  • In the absence of Medical Director, provides guidance to other medical personnel and center colleagues.
  • Perform all other related duties as assigned.

Qualifications :

  • Assures personal compliance with licensing, certification and accrediting bodies.
  • Licensure requirements of the state of jurisdiction
  • Graduation from an accredited Nurse Practitioner or Physician Assistant program
  • Procedural skills are a plus
  • Prior primary care exerience and experience as a visiting nurse a plus
