What are the responsibilities and job description for the MHCT/MCO Program Manager position at PHMC?

The Program Manager will be responsible for overseeing the operations of the Managed Care Organizations contracts in the Family Services Department and the operations of the Medical Home Community Team (MHCT) which operates in several counties of Pennsylvania. MHCT is a home-visiting service aimed at supporting families in overcoming barriers to comprehensive care and addressing social determinants of health and child development. MHCT provides assistance to parents and children, both with and without special healthcare needs, ranging from ages 0 to 21 years old in the form of care coordination and case management. This program operates with funding from MCOs. The Program Manager will need to frequently engage with funders, medical practices, providers, and members/clients associated with these MCOs.
Responsibilities:
Provides daily management of program activities in relation to MCO contracts
Works with the Assistant Director of Family Services to coordinate and implement contractual MCO projects, ensuring timely program deliverables are met.
Establishes and maintains strong relationships with funders, partners, providers, and project staff to ensure effective communication and efficient program implementation.
Initiates connections with community partners throughout contracted Pennsylvania Counties.
Collaborates with medical homes staff/providers, advisory committees, and referral sources to assess and determine the most appropriate practices and resources.
Works closely with MCO providers to enhance their capacity to meet the needs of their members effectively.
Coordinates and conducts all intakes with newly-referred families, assessing both child and family needs.
Ensures accurate and timely submission of program data and paperwork by staff, reviewing paperwork before completing data entry.
Works with the Assistant Director and MCO Coordinator to ensure goals and deadlines are met, assisting with evaluations of service effectiveness.
Completes program reports and other written documentation, maintaining accurate records according to grant funding and observing HIPAA requirements.
Attends required monthly MCO contractual meetings and partnered community-based county meetings.
Stays updated on current work in the community-based services field and shares information and resources with staff as appropriate.
Reviews and integrates new ideas and concepts with the Assistant Director to improve project delivery, content, and/or evaluation for the target audience.
Participates in and initiates conversations that introduce new ideas and concepts aligned with HPC's strategic plan in regularly scheduled team meetings.
Ensures educational materials are culturally appropriate for diverse populations, especially those with limited reading and English skills.
Represents MHCT and HPC at various meetings.
Administrative Activities:
Work with the Assistant Director and members of the MHCT Team on the implementation of new approaches to improve program delivery content and/or evaluation
.provides supervision, mentoring, training and orientation for MHCT staff including the MCO coordinator and the Community Health Navigators.
Identify opportunities for program expansion.
Collaborate with the MCO Coordinator for MHCT/Safe Care program enrollment.
Oversee project team regarding opportunities and plan for follow-up.
Document program activities in accordance with direction/protocol to support strong project data collection and/or publication opportunities.
Prepare reports about the MHCT program in collaboration with the Assistant Director and the MHCT Coordinator.
Support the Assistant Director with contractual and grant-funded obligations to ensure overall goals, objectives and deliverables are met.
Convene and facilitate weekly team meetings, with the support of the Assistant Director.
Carry a small caseload of the most intensively needy families.
Guide staffing defining priorities, deadlines, opportunities for growth, and resolving any program challenges.
Assist with publicizing program and program outcomes, including abstracts, posters, and presentations.
Participate in case conferences, trainings, case reviews, staff meetings, community fairs, etc., as required.
Establish and maintain linkages with medical providers, community health, social service, education, and legal service agencies, and other support services through effective communication between MHCT staff, community members, and MHCT partners.
Lead tasks related to material and supply inventory and other programmatic expenses.
Communicate formally on a bi-weekly or weekly basis with the Assistant Director.
Attend local and regional meetings and trainings, as needed.
Perform other relevant duties as assigned.

Must clear child abuse, criminal history check, and FBI clearance.
Must travel in Pennsylvania.
Ability to independently lead a small team.
Proficient in motivational interviewing techniques.
Ability to acquire information about new systems, organizations, and practices.
Experience:
At least three years of experience working with diverse populations and low-income individuals.
Minimum of three years of direct experience in in-home, community-based, and/or clinical services.
Minimum of two years as a supervisor.
Knowledge of issues related to maternal and child health, child development, child abuse prevention, and children with special healthcare needs.
Experience in data collection/entry, evaluation monitoring and needs assessments.
Preferred:
Home visiting experience.
Excellent understanding of the community-based organizations throughout Pennsylvania.
Knowledge of public health theories, principles, and practices.
Education Requirement:
Master's degree in social work preferred, bachelors degree in social work, public health or related field with a minimum of 5 years experience acceptable
Salary: Grade 19
