Responsibilities for a Social Worker with Phoebe Allentown:

• Assists residents on assigned units by providing information and referral services according to residents' needs; maintains contact with area social service agencies in order to obtain information and services for residents; serves as a liaison between the facility and residents' families and/or concerned parties.
• Performs social service assessments on residents in assigned units.
• Sets goals for residents and documents progress as well as contact made with residents and their representatives in accordance with state regulations; prepares a discharge plan for each resident as appropriate.
• Performs proper charting and documentation duties to ensure compliance with relevant state and federal regulations.
• Preforms tasks which are supportive in nature to the essential functions of the position, but may be altered or redesigned depending upon individual circumstances.

Requirements for a Social Worker with Phoebe Allentown:

• Bachelor's degree in Social Work, Social Welfare or MSW degree.
• Working knowledge of Pennsylvania Department of Health long-term care regulations.
• Minimum of 1 year of social work and /or related experience in assisting geriatric clients. Home & Community based service experience and dementia training preferred.
