Phoenix Behavioral Health is seeking full-time Licensed Clinicians/Licensed Social Workers to join our team! Position: Primary Outpatient Therapist
Department: Behavioral Health
Position Classification: Exempt
Reports to: Executive Director


Position Overview: Oversee day to day patient care for identified case load (across SUD/MH outpatient PHP/IOP service lines) individual, group, and family therapy services, coordination of treatment, referral source outreach, and collateral contacts, treatment planning, case management, biopsychosocial assessment, and NJSAMS assessment to ensure the patient enjoys a cohesive treatment experience where all are working together toward the same goal. Will attend and actively participate in weekly treatment team meetings, weekly staff meetings, and other meetings or responsibilities as assigned.

Demonstrate the ability to recognize and shift priorities and can easily welcome new company initiatives. Displays advanced knowledge of individual and group behavior and the effective ways of working with people to produce on-going recovery, being able to utilize a variety of evidence-based approaches to achieve the desired patient outcomes.

Specific Responsibilities:

• Maintains appointed continuous caseload (that modulates) by providing individual, family and group counseling, crisis intervention, as well as education and skills training (PHP, IOP services).
• Completes biopsychosocial intake assessments and appropriately determines level of care per the NJSAM or Phoenix Behavioral Health’s biopsychosocial other identified criteria.
• Completes NJSAM on MA SUD patients within 5 days of admission.
• Monitors oversight of client’s substance use (addressing with client and in collateral documentation) by overseeing random drug screenings and addressing results with clients per policy & procedure.
• Collaboratively develops and updates an individualized treatment plan (within 72 hours of initial admit and then every 30 days for updates) with the individual that includes all identified problems, goals, and discharge planning.
• Defines all relevant community sources affecting care and treatment of the client (e.g. legal, family, outside providers.) and communicates with those sources to coordinate care and ensure continuity of car.
• Update referral sources, primary care doctor/other providers, and family members on patient progress, if/as approved by the individual in services. Ensures releases of information is obtained on and on file for each referent or family member.
• Utilizes and incorporates evidenced based practices and standardized screening tools into treatment to engage clients in services and facilitate progress in treatment. (CSRRS, PHQ 9, etc)
• Maintains client electronic health record with required documentation per company and regulatory requirements. Ensures documentation of group note, individual note, case management notes, are timely and updated in DAP format.
• Meets ongoing caseload requirements by ensuring strong patient engagement that supports daily billable service requirements.
• Provides ongoing coverage to groups or takes on additional assignments as needed.
• Completes scheduled or urgent assessments as assigned.

Minimum Qualifications:
• A Master’s degree in Social Work, Mental Health Counseling, Addictions, Psychology or related field.
• License to practice independently preferred (LPC, LCSW)
• LAC, LSW, LCADC desired.
• At least 2-3 years of experience in a substance abuse and mental health setting serving culturally diverse populations.
• Advanced knowledge of the basic principles, philosophies, practices, and procedures involved in substance abuse treatment, including the biopsychosocial characteristics and manifestations of chemical dependency and/or mental illness.
• Ability to demonstrate competency in medical record documentation, specifically DAP group and individual notes to support utilization reviews.
• Demonstrates knowledge and ability to apply the NJSAM Criteria, DSM-V, and other standard screening and treatment related tools for placement and movement along the continuum of care.
• Must be able to work in a fast-paced environment with demonstrated ability to manage multiple competing tasks to meet deadlines and stay organized.
• Working knowledge of State and Federal statues regarding confidentiality laws and applicable regulations and standards.
• Must have strong knowledge of a variety of computer software applications in Microsoft work and Google platforms.


Work Environment: This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. The noise level in the work environment is usually moderate.


Physical Demands: While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands to finger, handle or feel objects, tools or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move objects up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision and the ability to adjust focus.
