Job Overview:
As the Partial Care Lead Case Manager, you will be the driving force behind our partial care program, ensuring that our clients receive the highest quality of support and resources. This role requires a high-energy, compassionate individual who is deeply committed to support of the clients and eager to make a meaningful impact in the lives of those we serve.
Key Responsibilities:
- Program Leadership: Oversee the day-to-day operations of the partial care program, including coordinating and supervising a dedicated team of case managers and support staff. Lead by example, fostering a positive and collaborative work environment.
- Client Advocacy: Act as a passionate advocate for clients, ensuring their needs are met with empathy and respect. Develop and implement personalized care plans that promote recovery, growth, and well-being.
- Case Management: Conduct comprehensive assessments, create individualized treatment plans, and provide ongoing support to clients. Monitor progress, adjust plans as needed, and ensure seamless communication between clients, families, and service providers.
- Team Collaboration: Work closely with multidisciplinary teams, including mental health professionals, healthcare providers, and community organizations. Promote effective communication and coordination to enhance client outcomes.
- Program Development: Contribute to the development and improvement of program policies and procedures. Stay informed about best practices in social work and partial care services, and integrate innovative approaches into the program.
- Community Engagement: Build and maintain strong relationships with community resources and organizations. Represent the program at community events and advocate for the needs of our clients within the broader community.
Qualifications:
Bachelor Degree preferred in social services or a related field.
Preferred LAC, LCSW
Preferred two years supervisory experience
