The Phoenix Center for Rehab & Pediatrics is a skilled nursing facility located in Haskell, NJ caring for both the Pediatric and Geriatric populations. We are looking for a devoted and caring Physical Therapy Assistant to join our team!
The PTA’s overall responsibility is to perform physical therapy procedures and related tasks that have been selected and delegated by the Physical Therapist. Responsibilities :
- Responsible to c ollaborate with the Physical Therapist and follow established Plan of Care for all patients
- Provide quality therapy services to patients who require services related to their medical diagnosis and physical impairment
- Monitor and document patient progress
- Assist PT in developing treatment plans and techniques to implement those plans
- Contribute to the resident’s assessment under the supervision of the PT
- Monitor resident’s response to treatment and report them to the PT and to other team members when so directed by PT
- Implement treatment in accordance with plan of care established by the supervising PT
- Document and maintain service-related records in accordance to Medicare, Medicaid, HMO’s as directed by PT
- Provide in-service training to nursing staff / families
- Communicate effectively with other therapists and departments on common issues, D / C plans and potential change in status under the supervision of the PT
- Perform any other related duties as deemed necessary
