Position Summary
Posting Note : The Phoenix Children's "Connect Care Center" is hiring a Part Time Patient Placement Coordinator. In this role, you will be providing clinical nursing guidance, and triaging patients on the type of care the hospital offers in all the areas we specialize in. You will also be working with Physicians from all over the country in educating them about the various services we offer our patients. In this unique role, you will work r 2, 8 hour shift (16 hours / week or 32 hours every pay period). The scheduled shift is 11 PM - 7 AM. There is a weekend shift requirement, and that is every third weekend. Phoenix Children's does offer a 15% night shift differential. And the weekend shift differential is an additional $2.50 / hr. This role is critical in managing the patients who are being admitted or discharged to the hospital.
This position is accountable for coordination and acceptance of information regarding direct admissions and transfers to Phoenix Children`s Hospital. Responsibilities also include coordinating all aspects of patient placement within the facilities, and partnering with nursing and other clinical operation departments to facilitate patient access, throughput, and capacity.
Position Duties
- Manages requests for all transfers and direct admissions, facilitates provider communication.
1. Accurately collects the data required for the intake of patients from a variety of locations including : ED, physician office, PCH clinic, referring facilities.
2. Coordinates all MD to MD acceptance calls; participates in and documents all acceptance calls, with an ongoing focus on quality and customer service.
3. Collaborates with Patient Placement Representative with determining patient benefits / eligibility.
4. Manages and accepts all ED referral calls (non-EMS) and communicates incoming referrals to ED.
5. Demonstrates the ability to triage and prioritize patient information and acuity utilizing critical thinking, clinical decision making, and established process flow to assist in assessing and facilitating patient flow across multiple facilities.
6. Demonstrates knowledge of human growth and development, assessment, and care of patients of all ages served.
1. Manages bed board on an ongoing basis in a timely and accurate manner.
2. Escalates bed placement / capacity issues to appropriate leadership in a timely manner.
3. Works with nursing units and leaders to identify best available bed placement at the time of request, given patient needs, staffing resources, level of care requirements, infection control principles, and throughput goals.
4. Reviews and plans for all scheduled admissions daily.
5. Collaborates with key departments such as admitting, EVS, case management to ensure timely patient admissions and a positive patient family experience.
