Posting Note: We are seeking an experienced Social Services Care Coordinator with a strong background in mental health and outreach services. The ideal candidate will have proven expertise in care coordination, case management, and experience working with DCS and vulnerable populations, including individuals with mental illness and intellectual disabilities, all in need of community support.

This is a fully remote position, but occasional in-person attendance is mandatory for meetings and team collaboration.

The Social Service Care Coordinator (SSCC)- is a part of the Phoenix Children`s Care Network (PCCN) Integrated Care Coordination (ICC) Team. The SSCC performs a wide range of duties that promote a positive patient-family experience; ensuring families have access to necessary resources while building collaborative relationships with local social service agencies, school districts, and mental health providers. The SSCC exhibits stellar customer service and problem-solving skills. The SSCC provides outreach directly to patients and families to identify gaps in care and current social service needs. This position acts as an advocate for patients and families, provides education about community programs, and assists with referrals and scheduling services. The SSCC performs BH post discharge ED and IP follow-up, ensuring a seamless return to home environment with appropriate services. This role serves as a subject matter expert on topics such as navigating the mental health system, locating community resources, and finding contracted mental health provider agencies. The SSCC interacts regularly with the ICC team`s Sr and Lead SSCC.

Position Duties

  • Social Service Activities
    a) Provides effective outreach to assess current needs and assist families with resources including information on where to obtain basic necessities such as clothing or food, specialty clinic information such as free and low-cost clinics for adult dental needs, WIC information, housing resources, mental health resources, school accommodation resources, and other social service agency information.
    b) Provides personalized education and resources regarding condition management, crisis intervention, parenting resources and support, etc. Will utilize PCH Emily Center or other reputable entities for basic, vetted mental health resources/education.
    c) Performs BH post discharge ED and IP follow-up, ensuring a seamless return to home environment with appropriate services
    d) Assists patients-families to schedule needed appointments with providers and community agencies.
    e) Identities and close gaps in care, address barriers, and to facilitates calls to providers and payers.
    f) Facilitates referrals and advocate for timely access to care, including sending medical records and other documents to carious community agencies and provide offices.
    g) Provides payer contracted provider lists to patients, families, and providers
    h) Provide general info and guidance about state and community programs
    i) Sets-up/participates in IEP and 504development or planning as needed
    j) Assists families with navigating the mental health, school system, or other related systems for patients with complex social and mental health needs
    k) Seeks guidance when hitting barriers to care. Appropriately escalating DTO/DTS to Lead SSCC and/or manager.
    l) Documents all care coordination activity in Pediatric Care Constellation (PCC)
  • Collaboration
    a) Coordinate and serve as a resource to the ICC team
    b) Assists Lead SSCC with ongoing training/educating team on basic social service/mental health needs.
    c) Assists in role based training of new staff
    d) Will be subject matter expert on hard to find or rare resources
    e) Attends meetings with other SSCC’s and manager
  • Customer service
    a) Responds to calls/voicemails/emails/IM within 24 hours
    b) Provides clear and consistent communication with both internal and external stakeholders
    c) Is willing to continue to hone skills for patient and family engagement strategies
    d) Able to multi-task using multiple monitors, email, IM, Vocera, PCC, etc.
  • Administrative
    a. Participates in keeping assigned in-house resource database updated regularly
    b. Participates in arranging community provider presentations for the ICC team
  • Performs miscellaneous job related duties as requested.
