Direct Support Professionals exhibit compassion, support clients, and ensure a safe environment. They foster independence and personal choice by assisting clients in acquiring necessary skills, such as social behavior and independent living skills so they have greater self-reliance in their day-to-day lives.
Requirements:
Must be 18 years of age or older.
- Valid driver’s license, reliable transportation, and good driving record.
- Must have the ability to interpret and implement all plans: PCISP, Behavior Plan, Health Risk Management Plan, Medication Administration
- Demonstrate the ability to communicate effectively and be able to follow all written and oral instructions.
- Must have completed all areas of required training with a competency score of 80% or above.
- Obtain Medication Administration Certification with the PCS Nurse
- CPR training must stay current
- Must have negative TB or chest x-ray.
- Must meet all requirements of the Criminal Background checks
Specific Responsibilities:
Direct Support Professionals must complete all required documentation at the end of every scheduled shift as outlined in the employee handbook.
· Monitors, reports, and documents all needs, changes, and conditions of the clients.
· Delivers individualized care for clients in compliance with Person-Centered Individual Support Plans (PCISP), Behavior Support Plans (BSP), Health Risk Plans, and Medication Administration.
- DSP adheres to all HIPAA regulations and promptly reports any violations to their supervisor.
· Assists clients in their daily living activities, which include bathing, dressing, toileting, and other domestic tasks. Additionally, you’ll handle home maintenance duties such as cleaning, laundry, and routine housekeeping.
· Operate PCS vehicles in compliance with company policies and governmental statutes.
· Transport and accompany clients to appointments, outings, and shopping as needed
- Assists, accompanies, and documents the client’s participation in community outings and recreational experiences.
· Create a safe and supportive environment that caters to client’s needs, self-expression, and aspirations. Additionally, ensure the protection and respect of the client’s home and personal belongings.
- Ensures the rights of the clients are protected and exercised at all times.
- Observes and documents the clients’ health needs as well as any incidents and promptly reports to supervisor.
· Maintain PCS medication certification through PCS Nurse
- Educates the client on medication and side effects as ordered by physicians or assists the client if self-medication has been approved
- During each shift, ensure that active client education is completed and documented. This includes discussing the consequences of client choices, promoting healthy eating habits, and emphasizing proper hygiene.
- Ensures compliance with all Policies and Procedures of Phoenix Community Services.
- Ensures compliance with all Federal and States Regulations.
- Attends Scheduled In-Service Training and home meetings as required
- Complete all other duties as assigned.
