Social Work Case Management Job Description

We are seeking a skilled Social Work Case Manager to join our team for a travel job in Auburn, Washington. This is a 13-week contract with 40 hours per week and 8-hour shifts, Monday through Friday.

Key Responsibilities:

• Provide case management services to clients with complex needs.
• Develop and implement individualized care plans to meet the unique needs of each client.
• Collaborate with healthcare professionals, family members, and community resources to ensure comprehensive care.
• Conduct regular assessments and evaluations to monitor client progress and adjust care plans as needed.

Requirements:

• Masters or Doctoral degree in Social Work (MSW or DSW)
• Current LICSW and Washington state license
• Minimum 3 years of experience, with at least 1 year in an Emergency Department setting
