Social Services Assistant

Piatt County Nursing Home is currently seeking a Social Services Assistant.

JOB SUMMARY

The Social Service Assistant plans and implements a wide range of social service functions designed to support the philosophy of Piatt County Nursing Home and the care planning needs of the resident. The Social Service Assistant serves as a liaison between the resident, resident family, facility and the community.

RESPONSIBLE TO

The Social Service Assistant is responsible to and reports directly to the Social Service Director.

JOB PERFORMANCE REQUIREMENTS

• Assesses the need for social services at Piatt County Nursing Home and participates in the development of programs and services to meet those needs. Serves as a resident’s advocate fostering human dignity and the personal worth of each person.

• Completes a social history and assessment for each resident within fourteen days of admission. This social service information is obtained from the resident and/or family member, referral source, or appropriate social service agency.
• Provides hands-on implementation of approved recreational/therapeutic programs within the facility as needed.
• Orients new residents and family members to the facility. Assist the resident’s family in developing constructive and meaningful ways to support the resident’s experience within the facility.
• Serves as a companion, listener and mediator in resident relationships.
• Serves as a teacher, listener and mediator to families in their relationship with their resident and the facility.
• Effectively interacts with community members and volunteers displaying accountability and good judgement, e.g. recognizing community member/volunteers gift of time to the facility.
• Informs the resident, guardian, or resident’s representative of the resident’s rights and responsibilities while living in the facility.
• Provides counseling to residents, family members, and resident representatives at the direction of the Social Service Director.
• As “Co-Discharge” planner, actively participates in the facility’s discharge planning process and assures facility compliance with discharge regulations.
• Arranges transportation for medical, dental and other services required by the resident outside of the facility. Note: May serve as the driver for transportation as needed.
• In conjunction with the Accounting Coordinator, assists the residents in maintaining personal funds within the facility.
• Assists with shopping and personal errands for the residents weekly.
• In conjunction with the County Clerk’s office, assures that residents are allowed to exercise voting rights in national, state and local elections. Maintains compliance with the Illinois Election Code, as it pertains to voting rights and absentee voting.
• Identifies social service needs of the residents through Social Service Progress Notes, reflecting goals, intervention provided, and the response of the resident.
• May be requested to meet with various community organizations and agencies to facilitate cooperative efforts between Piatt County Nursing Home and the community.
• May be assigned to assist in supervision of resident personal laundry services. Orders name tags and maintains program labeling resident clothing. Resolves resident problems concerning personal clothing.
• May be assigned to complete personal clothing inventory sheet for all new residents and updates annually. Maintains program for labeling glasses, denture, hearing aids etc.
• Establishes and maintains Social Services procedures to ensure the confidentiality of resident information.
• Attends staff development programs as requested by the Director of Social Services and continually updates self-knowledge of current literature and new developments in the delivery of Social Service/Activity programs.
• Maintains good working relationships with other departments and participates on facility committees as assigned.
• Is responsible for being knowledgeable of and able to perform all Fire and Safety, Tornado, and Disaster Evacuation procedures. Participates in all building preparedness programs.
• Is familiar with Operating Policies of Piatt County Nursing Home.
• Supports the Goals and objectives of Piatt County Nursing Home and administers the Social Services Department in compliance with established facility policy. Supports, honors, and fully abides by the Standards of Ethics and Integrity of Piatt County Nursing Home.
• Performs other duties as required by the Director of Social Services.

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
