TITLE : ATP Counselor
REPORTS TO : Unit Supervisor
REQUIREMENTS : Bachelor’s degree
Fingerprint Clearance
Able to walk up three (3) flights of stairs
PRIMARY TASKS AND RESPONSIBILITIES :
- Maintain primary responsibility and accountability for assigned caseload of clients. “Primary responsibility” means provision of needed services within residential program setting.
- Record hours in ADP daily and submit requests for time off through ADP.
- Assessment of residents in terms of functional abilities and service needs.
- Development of service plans, with residents’ input, aimed at assisting residents establish and accomplish person goals.
- Provide and document provision of six (6) restorative services monthly.
- Provision of case management including, but not limited to, necessary linkages to needed / desired services (e.g. day programs, vocational rehabilitation services, legal services, medical / dental services, outpatient psychiatric services, etc.) and attending case conferences focusing on coordination of treatment / services provided to residents.
- Assist / teach residents daily living / social skills including, but not limited to, budgeting, personal hygiene, maintenance of living space, shopping, traveling, meal planning / preparation, use of leisure time through participation in recreational and social activities, facilitating participation in available program groups.
- Facilitate interaction with clients enabling the development of a therapeutic alliance.
- Work with clients on discharge planning from date of admission.
- Document all interactions with clients through progress notes.
- Attend unit meetings as scheduled.
- Attend weekly supervision with Unit Supervisor.
- Report any changes in clients’ behavior immediately to Unit Supervisor or Program Director.
- Rotate through exterminator schedule.
- Available to work longer hours when needed in emergency situations.
- Follow all agency ethics and conduct.
- Inform Program and Quality Assurance Directors of events that constitute incidents.
- Perform other necessary duties assigned by Unit Supervisor and Program Director.
- Participate in annual anti-sexual harassment training.
