What are the responsibilities and job description for the ACT Nurse Case Manager position at Piedmont Community Services?
Assertive Community Treatment Psychiatric Nurse needed for the ACT team to work with a team of professionals in assisting individuals diagnosed with a serious mental illness to remain successful in the community. This position is responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist. Providing a range of treatment, rehabilitation, and support services; and sharing shift-management responsibility with other staff. Provides direct case management services to individuals, conducts medication education, and performs necessary documentation. Assists individuals with weekly medication bag/box fills. Delivers medications to the homes of individuals per morning/evening medication runs on a rotating basis. Some evening, weekend and holiday hours required. Registered Nursing degree preferred with at a minimum of one year experience working with adults with severe mental illness. Must have a minimum of a licensed practical nurse degree with at least three years of experience in the provision of mental health services to adults with serious mental illness. Salary is commensurate with education and experience as it relates to the position.
