What are the responsibilities and job description for the School Based Mental Health Case Manager position at Piedmont Community Services?
Provide case management to children at Stuart Elementary School in Patrick County. Position is responsible for ensuring that youth receive services to assist them in accessing the needed medical, psychiatric, social, education and other supports essential to meeting their basic needs. Qualifications: Bachelor's Degree in human services or related field is preferred with one year experience of direct care or any equivalent combination of training, skills and abilities as defined by DMAS. Flexible schedules are optional.
