Job Title - Geriatric Adult Nurse Practitioner
Department - PHSC
Reports to - PACE Medical Director
Benefits -
Education: Graduate of certified or accredited Nurse Practitioner Program.
Required: 5 years clinical nursing experience with at least 2 years supervisory experience.
Preferred: One year of experience with the frail or elderly preferred.
Current/valid License: Certification from Nurse Practitioner program; valid NC Driver License.
Immunizations: All PHS Required Immunizations
Pay Range - $104,019 - $120,586

Job Title - Geriatric Adult Nurse Practitioner

- Schedule: Monday - Friday 8:00am - 5:00 pm (Requires call every 7-8 weeks)
- Travel: As needed
- Performs health histories, physical exams and patient teaching.
- Orders, interprets and follows-up on lab test results.
- Performs selected procedures and documents are orally and in writing.
- Initiates emergency measures as indicated.
- Recognizes abnormalities that justify immediate referral for intervention by others.
- Initiates, evaluates, reviews and changes orders for selected medications and diagnostic studies/treatments within attend physician approved protocols.
- Assesses initiates and coordinates patient care on a daily basis under the supervision of the attending physician.
- Conducts first post-enrollment visit.
- Coordinates referral documents.
- Communicates and coordinates referral documents to medical specialists, hospitals and nursing homes.
- Facilitates pre-op paperwork for labs, EKG’s, x-rays and hospital preadmissions.
- Coordinates post-discharge facility, rehab and Plan of Care changes related to hospitalizations.
- Coordinator of nursing and assisted living facilities.
- Requests consultations from all services as needed in collaboration with attending physician and/or established protocols.
- Acts as liaison with physicians, nurses and other healthcare professionals within the program, providing education, communication and support for healthcare staff, patients and families.
- Directs patient management in conjunction with physician staff.
- Participates as a member of IDT, attends staff meeting and takes part in participant care planning.
- Communicates any changes in participant condition to team members.
