Job Title - Lead Agricultural Worker Health Specialist
Department - Health Support Services
Reports to - Agricultural Work Health Manager
Benefits -
Performs outreach tasks, such as evaluations, referrals, and follow ups as needed.
Develops and maintains relationships with key individuals and agencies in the community.
Provides direct services: such as transportation, interpretation, food, clothing and toiletries.
Uses health assessments to evaluate health risks and other social needs/follow ups.
Distributes information to farmworkers regarding clinic services, and other state/locals services.
Maintains current information on camps/housing locations, demographics, and health status on farmworker communities. Maintains outreach supplies.
Documents properly and accurately all patients seen within the program.
Contacts other health professionals or agencies if needed to provide optimal care for the patient.
Works with other health center staff or outside medical professionals to provide care for patients.
Provides case management services (such as assessment, referral, follow up and reassessment when needed).
Demonstrates the ability to work with diverse patient populations served and has a commitment to advocacy and cultural diversity.
Qualifications -
Education: Minimum requirement is high school diploma, preferred associate or bachelor’s degree or equivalent experience.
Required: Bilingual English/Spanish required
One year of public health outreach experience.
Preferred: Preferably experience related to migrant farmworker populations.
Current/valid License: Driver’s License
Immunizations: All PHS Required Immunizations
Pay Range - $46,786 - $62,876
