The Nurse Practitioner is responsible for applying advanced knowledge and clinical skills to assess and manage the healthcare of geriatric participants at Piedmont Health SeniorCare facilities. Piedmont Health SeniorCare is a Program of All-inclusive Care for the Elderly (PACE).
The Nurse Practitioner performs physical assessments and cognitive evaluations, diagnoses and manages acute and chronic health conditions, prescribes medications and develops individualized treatment plans, and provides guidance on preventive care, nutrition, mobility, and fall prevention to all participants. This role coordinates with families, caregivers, and interdisciplinary care teams to create and implement care plans, offers counseling and support around advanced directives, palliative care, and end-of-life decisions. This role monitors and adjusts treatment plans based on participant responses and changing health statuses, and facilitates referrals to specialists as indicated. This position educates participants and caregivers about disease processes, medications, and self-care strategies.
The Nurse Practitioner works collaboratively with physicians, social workers, behavioral health providers, therapists, nutritionists, and care teams to promote healthy aging and improve the quality of life for our participants.

- Schedule: Monday - Friday, 8:00am - 5:00pm
Qualifications -
Education:
- Master’s degree in Nursing from an accredited institution;
Experience:
- 5 years of experience in clinical nursing;
- 1 year of experience serving geriatric populations, preferred;
- Experience with underserved populations from diverse cultural backgrounds, preferred;
License and Certifications:
- Current, unrestricted North Carolina license to practice as a Nurse Practitioner;
- Board certification from either American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP);
- Controlled Substance Registration Certificate from the Drug Enforcement Administration;
- Basic Life Support (BLS) certification;
Knowledge, Skills & Abilities:
- Strong clinical assessment and knowledge of geriatric syndromes and polypharmacy;
- Commitment to serving low-income, uninsured, and diverse communities;
- Excellent communication and interpersonal skills, and cultural competency skills;
- Ability to educate participants on health-related matters;
- Deep understanding of geriatric physical and cognitive changes;
- Effectively handle difficult and unusual interpersonal situations;
- Ability to learn, teach, and research;
- Great organizational skills;
- Ability to solve problems and think analytically/critically;
- Familiarity with quality reporting standards;
- Comfortable working with interdisciplinary teams;
- Ability to work well independently and collaboratively;
- Proficiency with EHR systems (Epic);
- Commitment to health equity, social justice, and community health;
- Ability to adapt to a fast-paced environment;
- Empathetic, respectful, and patient centered approach to care;
- Comfortable with end of life conversations and providing ethical decisions;
- Knowledge of NC Medicaid, Medicare, and community based long-term care services;
- Proficient in Microsoft Word, Excel, PowerPoint and Outlook.

Pay Range: $104,019 - $139,792
