About Position : A Lead Member Care Coordinator will provide clinical and administrative guidance and oversight for the team of member care coordinators. The Lead Member Care Coordinator is instrumental in providing training and expertise to the entire member care coordinator team. In addition, the Lead Member Care Coordinator performs member care coordinator duties as needed. The Lead Member Care Coordinator guides and helps implement population health initiatives to improve patient health outcomes. This position will engage in the development of policy guidelines and workflows. The Lead Member Care Coordinator will collaborate with multidisciplinary teams, community partners and other stakeholders as applicable. Performs other duties as assigned.

Qualifications -
Education : 2-3 years of care coordination experience within PHS.
Required : Proven success within quality areas such as chart audits and productivity.
Exhibits leadership qualities such as successfully facilitating meetings, consistent success with job duties, and consistent success within quality areas.
Preferred :
Current / valid License : N / A
Immunizations : All PHS required Immunizations
Pay Range - $50,306 - $58,319
