About Position : The primary responsibility of the Social Worker is to provide social work care management services to the participants of the Piedmont Health SeniorCare program; provides services such as participant screening, assessments, care management, supportive listening and coaching and referrals.

Qualifications –
Education : This position requires a master's degree from an accredited school of social work, 2 years professional level experience in a health related area.
Required : 2 years professional level experience in a health related area.
Preferred : (Bilingual) One year of experience with the frail or elderly preferred.
Current / valid License : MSW or LCSW. If not a licensed clinical social worker (LCSW), the incumbent must obtain NC Certified Master Social Worker within sixty days of employment. Immunizations : Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
