Role Description
This is a full-time on-site role for an RN - Care Manager located in Macon, GA. The RN - Care Manager will oversee the day-to-day care management of patients, including coordinating and navigating patient care while ensuring the highest quality of clinical standards are maintained. Responsibilities include patient assessments, coordinating treatment plans, managing patient care transitions, and collaborating closely with medical staff and other healthcare providers to ensure optimal patient outcomes.
Qualifications
MINIMUM EDUCATION REQUIRED:
Graduate of Nursing Program
MINIMUM EXPERIENCE REQUIRED:
Two (2) years of experience in an acute or post-acute setting
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current unrestricted registered nurse (RN) license, or eligible for licensure, in the state of Georgia, required.
ADDITIONAL QUALIFICATIONS:
Certification in Case or Care Management preferred. Experience using InterQual or Milliman preferred. Masters degree preferred.
