How to be successful in the role : You will join the Family Caregiver Support team with the ability to quickly learn and appropriately deploy community-based resources to support informal / family caregivers caring for individuals living in their own homes. You will develop the skills to evaluate the needs of caregivers and their care recipients and determine the right federal, state, and local social service programs that will help them remain in their own homes for as long as possible. You will also receive training from peers and supervision to assist with meeting the position goals. Position Summary Core Daily Responsibilities : Authorization, oversight and monitoring of older and disabled client care plans, along with prompt response to changes in client's condition and care needs.Provide comprehensive assessment to determine any individualized client needs using a laptop in the client's home settingRespond to individuals seeking information by providing routine information, referrals and assistance.Monitor both the progress of the client and the adequacy of the services being provided.Provide coordination of services and advocacy for older and disabled adult clients.Core Strategic ResponsibilitiesCompletion of care recipient and caregiver assessments, home visits, regular care plan monitoring contacts.Prompt response to client-related phone calls and emails, generally within 2 business days, which aligns with the Human Services Strategic Plan.Frequent communication with clients, family members and community providers to ensure services are being provided in accordance with the established care plan.This is a "summary" of the duties.
