Job Title: Social Worker (LCSW) – Case Manager (Hospital, Inpatient)
Location: Pittsfield, MA
Duration: 13 Weeks with possible extension
Shift: Day Shift (8:00 AM - 4:30 PM)
Hours per Week: 40-hour guarantee
Responsibilities:
- Conduct comprehensive admission and discharge risk assessments related to quality, financial, payer benefit allowances, and limitations.
- Specialize in utilization management, including levels of care knowledge and competence with evidence-based criteria tools.
- Collaborate with providers, patients, families, insurance organizations, and government agencies to coordinate effective care management.
- Ensure compliance with CMS regulations, appeal rights, and DPH regulations.
- Maintain knowledge of Kepro QIO (appeals) and other regulatory/accreditation organizations such as Joint Commission, DPH, and HealthGrades.
- Work independently to evaluate patient needs and plan for effective discharge planning and care transitions.
- Utilize data management, research, analysis, and reporting skills to prepare comprehensive summary reports when necessary.
- Maintain confidentiality and demonstrate excellent interpersonal, communication, and negotiation skills.
- Establish and maintain relationships with physicians and local community resources.
- Use multiple computer applications to manage patient data effectively.
Qualifications & Requirements:
- LCSW - Licensed Clinical Social Worker – Required
- Active Massachusetts License – Required
- BLS (AHA) – Required
- 3 years of experience in acute care
- Prior experience in care management or third-party insurance review – Preferred
- Proficiency in CMS regulations, appeal rights, and DPH regulations – Required

Pay: $2,754.45 - $3,044.97 per week
