Discharge Planner Role
The LVN Discharge Planner plays a crucial role in ensuring a smooth transition of patients from acute care to the next level of care and services. This involves collaboratively determining the level of care needs beyond acute care, providing decision support to patients and families, as well as physicians.
This position requires strong verbal, written, and organizational skills, along with a broad knowledge of disease processes, pathophysiology, and treatment plans.
- Key Responsibilities:
- Provide discharge planning by determining level of care needs beyond acute care.
- Support patients and families in making informed decisions about their healthcare.
- Ensure a smooth transition to the next level of care and services.
Requirements:
- Maintain an active California LVN license.
- BLS certification from the American Heart Association.
- Minimum of 2 years' acute hospital experience.
- Computer proficiency.
Preferred Qualifications:
- Previous case management experience.
- Understanding of treatment plans and the healthcare delivery system.
