JOB SUMMARY:
The Patient Navigator will serve as a Patient Liaison to help patients and family members “navigate” PMC’s continuum of care after a patient receives a cancer diagnosis. The Patient Navigator will work with the Diagnostic Imaging Department, Pathology Department, Medical Oncology Physicians, Radiation Oncology Physicians, Surgeons, Social Work, Nutrition and Case Management to assure timely access and appropriate intervention during the cancer patient’s care. The Patient Navigator assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes for oncology patients.
ESSENTIAL FUNCTIONS:
1. Develop relationships with physicians and departmental staff to assist with access issues and to assure timely diagnosis and testing.
2. Keep communication open with providers and patients to coordinate patient care services.
3. Develop relationships with the American Cancer Society, Kentucky Cancer Control, support groups and other agencies to make appropriate referrals.
4. Engages the patient and the patient’s family to assure that they understand the process of cancer care.
5. Gathers information and directs patient to websites to gain educational information.
6. Regularly communicates with the patient during active care and at appropriate intervals after the completion of active care.
7. Assures access to supportive care.
8. Works cooperatively as a member of the oncology care team.
9. Maintains documentation, algorithms and a tickler file to assure timely follow-up and alerts.
10. Acts as a patient advocate.
11. Acts as a community health educator.
12. Adheres to dress code.
13. Adheres to medical center policies addressing infection control, safety, and personnel guidelines.
14. Demonstrates knowledge of and compliance with the medical center code of conduct and applicable corporate policies and procedures.
15. Maintains patient confidentiality and treats patients and their families with respect and dignity.
16. Functions as a preceptor to orient new employees.
17. Performs other related duties and assigned or requested.
JOB REQUIREMENTS:
1. Interpersonal skills.
2. Experience with working with oncology patients and families are preferred.
3. Knowledge of the breadth and accessibility of community resources.
4. Thorough knowledge of health care systems and terminology.
5. Working knowledge of HIPPA regulations.
6. Self directed and able to organize and manage multiple tasks/projects simultaneously.
7. Ability to promote programs and services to community.
8. Extensive understanding of oncology care.
9. Proficient in learning medical record systems (electronic and non-electronic).
10. Written and verbal communication skills.
EDUCATION AND LICENSURE:
An Associate’s Degree in Nursing is required. Bachelor’s Degree in Nursing is preferred. Current Kentucky RN license is required. BLS training is required. Three or more years of experience, preferable with oncology patients.
WORKING CONDITIONS:
Works in well-lighted business office environment the majority of the time
There is a potential for being at risk of exposure to hazardous chemicals when working. Information regarding hazards and protective measures along with training on handling and storage of hazardous materials will be provided prior to initial assignment to work with hazardous chemicals, whenever the hazard changes, and on an annual basis
