Population Health Data Analyst- Performance Improvement
Associate degree is required. Experience with EHR, data collection and analysis strongly preferred. Ambulatory healthcare experience preferred.
Strong knowledge of general office equipment and computers, including Word, Excel, and other Windows applications, is required. Must achieve and maintain strong working knowledge of the ambulatory EMR. Medical terminology is required. The ability to communicate effectively with physicians, nurses, other healthcare professionals is required. Strong computer skills is required.
The Population Health Data Analyst collects, monitors, analyzes, and reports data for the evaluation of population health and equity strategies. This position involves collaboration with physician practice leadership, quality teams, and organizational leadership to evaluate systems to meet the needs of the population served. Responsible for ongoing collection and monitoring of Medicaid HRIP (Hospital Rate Improvement Plan) and KTHRII (Kentucky Trauma Hospital Rate Improvement Initiative) data. Runs/analyzes reports for population health and works with physician practice resources to facilitate care gap workflows to ensure population health goals are met.
