Job Title : Social Worker- Early Intervention Program
Location : Northern New Jersey or Central New Jersey.
Summary :
We are seeking a dedicated Early Intervention Social Worker to provide family-focused support and educational services to infants and toddlers (birth to age three) and their families. Services will be delivered in the child’s natural environment, in collaboration with other practitioners and service coordinators, in accordance with the child / family’s Individualized Family Service Plan (IFSP).
Responsibilities :
- Conducting evaluations and ongoing assessments.
- Developing IFSP outcomes with families and service providers.
- Referring families for additional services, including therapies and medical evaluations.
- Assisting families in accessing Early Intervention and community resources.
- Completing required documentation in a timely manner.
- Collaborating with teams to monitor progress and plan future services.
- Adhering to safety protocols and maintaining confidentiality.
- Participating in training, workshops, and professional development.
