The Pima County Health Department is seeking a Tribal & Rural Case Manager to support indigenous and rural communities by improving health outcomes and reducing fatal overdoses. This role provides care coordination, referrals, screenings, and eligibility determination for medically necessary services while collaborating with Tribal entities, crisis teams, first responders, housing programs, and the justice system to ensure individuals receive the support they need.
This position involves outreach and follow-up for individuals at high risk of overdose, offering education on substance use services, naloxone administration, and harm reduction. You will help clients and families navigate the behavioral health system, advocate for them with service providers, and develop safety plans to connect them with treatment and recovery resources. Additionally, you will provide community education and training to increase awareness of overdose prevention, harm reduction, and stigma reduction, while working closely with Tribal and rural organizations to improve service access and identify gaps in care.
The ideal candidate is compassionate, detail-oriented, and highly organized, with strong interpersonal communication skills and cultural humility. Graduation from an accredited school of nursing AND two (2) years of professional nursing experience.
Or
Bachelor’s degree from an accredited college or university with a major in social or behavioral science, or a closely related field as defined by the appointing authority AND two (2) years of experience providing social services.
(Relevant experience and/or education from an accredited college or university may be substituted.)

Failure to obtain and maintain the required licenses and certifications shall be grounds for termination. Any offer of employment resulting from this recruitment is contingent upon Fleet Services’ review and approval of the candidate’s driving record.

- Experience with computer software, including Microsoft Office Products and electronic health records.
- Experience working collaboratively with community and multisectoral partners displaying strong partnership building skills.
