Speech Therapist

Job Description:

We are seeking a skilled Speech Therapist (SLP) to join our team at a local Skilled Nursing Facility.

Responsibilities:

• Collaborate with the therapy teams and Rehab Department to provide comprehensive speech therapy services.
• Evaluate patients and complete necessary documentation to support their care.
• Develop and implement treatment plans that address communication, assessment, and dysphagia needs.
• Provide speech therapy as outlined in treatment plans and prescriptions to improve patients' ability to communicate effectively.
• Manage workflow independently, including documenting individual and group treatments and notes in the electronic records system.
• Stay up-to-date on PDPM guidelines and ensure alignment with Medicaid and Medicare billing requirements.

Qualifications:

• New Jersey SLP License
• Experience working in long-term care, specifically in a Skilled Nursing Facility
• Strong organizational, interpersonal, and communication skills
• Ability to manage patients with diverse personalities and abilities
• Creative problem-solving skills
