Job Summary
The Social Services Coordinator at Pine Haven Christian Home will provide support for the emotional and psycho-social needs of our residents and their families. This individual's focus is to provide expertise to enhance the care we provide to our assisted living residents.
Essential duties and responsibilities
- Assist with psycho-social aspect of potential resident assessments
- Provide tours to potential residents and families
- Coordinate, and complete the admissions of new residents
- Responsible for providing residents and families with the social and emotional aspects of transitioning to our facility
- Meet with residents and families regarding emotional, social, or physical needs
- Assist staff with challenging resident or family situations
- Provide emotional support as resident needs arise
- Connect residents and families with community and government benefit programs and resources
- Coordinate, facilitate and participate in service plans, and complete follow up tasks from the care planning
- Investigate, document, and follow-up on resident rights concerns, including lost items
- Develop social services programs to provide higher level of care for residents, families, and staff
- Coordinate schedules for residents and outside providers who come on-site, such as podiatrist, audiologist, ophthalmologist / optometrist
- Work closely with support department staff and managers, including providing direction as needed to staff and sharing concerns with managers
- Serve on the Administrative On Call Rotation
- Provide Quality Assurance structure, review of programs, and implement QA initiatives as they are developed
- Follow infection control and universal precautions policies and procedures to ensure that a sanitary environment is always maintained
- Attend and participate in required training, conducted at time of hire and / or annually, that will include training designed to assure resident safety : i.e. fire safety, missing resident, evacuations, disaster response, etc.
- Report any observed changes in a resident's needs and / or condition immediately to a license nurse or manager as appropriate
