Nursing personnel must have the following:

  • High school diploma or GED
  • Graduation from a state approved/accredited nursing program and/or college or university
  • Experience in the oversight of LPN’s or
  • Valid LPN license in the State of Minnesota (in good standing)
  • Two (2) years of experience as a practicing LPN
  • Ability to administer prescribed medication and medical treatments following standard nursing procedures
  • Ability to assess and monitor physical and/or mental conditions of patients/clients
  • Ability to maintain effective communication with patients, families, and members of the agencies
  • Ability to provide documentation, prepare reports and maintain patient/client records, including computer skills
  • Ability to participate in defining, interpreting, and implementing standards, policies, and procedures which affect the delivery of nursing care services.
  • Ability to participate in regular meetings

The LPN:

a. Provide a copy of current license.

b. Ensure all clients have a current TB Risk Assessment done.

c. Develop a TB Infection Control Committee & TB risks level.

d. Provide annual TB education on TB exposure, risk factors, signs and symptoms of TB disease and TB infection control, policies, and procedures.

d. Staff will do full body checks which include skin checks, breast, and genitals of all clients on a semiannually basis.

e. Staff will conduct all Discus and Moses assessments.

f. Staff will attend all semiannual and annual team meetings for the clients.

g. Staff will review and sign all client’s monthly health documentation from appointments. Staff will bring any concerns to the Program Director.

h. Staff will fill out all MOSES and discus information at least semiannually for each client’s medication monitoring and more often if it is a new medication.

i. Provide competency checklists for Medication Administration, First Aide, AED, glucose monitoring (if applicable) and agency facility information, as required by the Agencies.

1. Staff will train staff on the health needs record, universal precautions, sanitary practice policy, TB training, safe medication assistance and administration policy, medication and treatment administration packet, health needs change notice, health service coordination and care policy.

j. Staff will take residents to get annual flu shots, record the needed information and turn the sheets into the main office building (consent from guardians & staff must be given in writing prior to flu shot given).

k. Staff will also review medication errors and review for patterns and speak with Program Director if concerns.

l. Staff will conduct monthly medication reviews on the MARS for each resident at Pine Ridge Residence Inc to ensure that the MARS matches up according to the previous month, ensure written orders are available before meds given in the MAR.

m. Staff will check BM status or protocols every month for all clients and address any concerns.

n. Ensure the staff fully complies with the policies and procedures of the agency to which they are assigned, the applicable standards of care, and all applicable regulations as now existing or as may be modified.

o. The staff shall respond in a timely manner to all medical concerns reported by Pine Ridge Staff.

p. The staff shall review the Pharmacy Tech Consultants notes and provide guidance to Program Director.

q. The staff shall meet with the Program Director on a monthly basis following their Medication Review to address any concerns or changes that need to be made.

r. The staff shall have contact with the clients at least once a month for the clients to have the opportunity to bring up any health concerns they have for themselves up to the nurse.

s. Provides any necessary treatments that are needed for the residents that DSP/DSLs are not trained or certified to perform.
