- Graduate of a CSWE accredited bachelor’s degree required
- Master’s degree preferred
- One-year experience of supervised work in a health care setting required
- Capacity to effectively communicate with and assess residents
- Responsible for assuring residents rights are met and residents, family members and team members on resident’s rights as necessary.
- Meets with resident/family upon admission to assess and plan to meet psychosocial needs.
- Provides supportive counseling as needed to adequately assist resident and family through the admission process.
- Participates in development and revision of resident handbook, resident newsletter and other community documents.
- Coaches and enables other team members to respond to resident’s emotional and/or psychological needs.
- Initiates and maintains relationships with resident and family that positively effects their morale.
- Orients residents and families to environment and assists in adjustment through involvement in both individual and group programs.
- Completes social history and assessment on each resident upon admission, if qualified.
- Completes appropriate documentation upon admission, transfer, discharge, and completes ongoing progress notes at intervals required by regulations.
- May support the resident admission process, in conjunction with the Marketing/ Admissions team members.
- Coordinates discharge planning process and arranges for resident services after discharge, including: DME; nursing care; home rehabilitation, etc.
- Maintains a current community resource file for education and communication of community resources, external referral sources to residents and families.
- Upon request of the Executive Director, provides social services for campus residents outside the Health Center.
- Actively participates, and encourages resident/family participation, in the Interdisciplinary Care Planning Conferences.
- Designs and conducts formal/informal education and/or counseling sessions (if licensed) for residents and family members and team members, including: intervention skills, re-motivation/ behavioral programming, orientation/coping skills and other appropriate modalities.
- Actively participates in the community Quality Assurance Program, developing and implementing performance improvement plans as warranted.
- Participates in other sub-committees as needed.
