Pine Valley is registered with Wisconsin DHS, our employees are eligible to apply for FREE CDCP certification (Certified Direct Care Professional). Training is provided through the State of Wisconsin with $500 in cash bonuses paid upon completion of the certification. Our schedules complement job sharing, college class schedules, work as a great second job and can decrease daycare needs / costs.

Responsibilities and Job Duties

Provide personal assistance, support, direction, care, and supervision to up to (4) clients in a smaller home-like environment.

Perform household tasks; cleaning, basic home related chores and assigned client care tasks throughout each shift.

Perform charting and documentation, medication assistance and monitoring for symptoms.

Prepare and serve (3) meals daily according to company menu. Prepare and submit food and supply order lists weekly.

Assist and supervise clients in learning Independent Living Skills.

Assist and direct clients in Activities of Daily Living.

Plan and participate in activities, plan weekly outings and crafts with clients in the home.

Work with the supervisory team to ensure compliance, safety and cleanliness in the home.

Work as a team with other direct care staff and supervisors.

POSITION REQUIREMENT :

Must pass criminal background check
