Summary
As the Outreach Manager at Pineland BHDD, you will be instrumental in expanding our community presence and engagement efforts. This role is vital in promoting our services and fostering partnerships that enhance the well-being of those we serve.
Responsibilities:
· This person will support Pineland BHDD veterans population receiving services as well as do outreach to ensure services are made known to veteran’s who need services but may not be aware of the services provided by the agency. Position will also support families of veterans and provide care coordination both internally and externally. This position serves as the Pineland BHDD contact for both internal and external veterans that may need behavioral health services. This position requires staff to have home visits to determine if veteran and family members are receiving the appropriate mix of services. Position makes referrals to both Pineland and community base programs as needed. This position involves tracking and reporting on high-risk Veterans, monitoring treatment services, providing consultation to providers, collaborating with community organizations, and promoting evidence-based interventions for suicide prevention
· The position requires comprehensive documentation of treatment plans and outreach efforts to ensure effective support for Veterans
· Coordinate care for Veterans identified as high-risk for suicide
Qualifications:
· English Language Proficiency: Candidates must be proficient in spoken and written English
· Veteran Status preferred
· BA in Psychology, Social Work, Criminal Justice, Education or similar field.
