Job Summary
This position is responsible for ensuring a seamless transition for patients coming into and out of acute care hospital settings. As an Intake Coordinator, you will be working closely with physicians, nurses, and other healthcare professionals to assess patients' needs and ensure they receive the best possible care.
Main Responsibilities
- Conduct thorough nursing assessments to identify patients' needs and develop individualized care plans.
- Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of treatment plans and discharge instructions.
- Coordinate with House Supervisors/Nursing Administration to prioritize workload and identify areas for improvement.
