The Home Health Aide (HHA) provides personal care and related services in the patient’s home under the direction, instruction, and supervision of a registered nurse (RN). All tasks performed by the HHA must be assigned by, and carried out under, the supervision of an RN who is responsible for the patient care provided.
- Adheres to the care plan to ensure safe and competent care for the patient.
- Assists patients with personal hygiene and supports the maintenance of a clean, safe, and healthy environment.
- Helps patients with ambulation as prescribed by the physician and approved or supervised by a nurse or therapist.
- Supports therapy personnel as needed during rehabilitative processes.
- Encourages patients to achieve maximum independence according to the nursing care plan.
- Promotes mental alertness by engaging patients in activities of interest.
- Provides emotional and psychological support to patients and household members, fostering trust and confidentiality.
- Reports any changes in the patient’s condition or home environment to the RN or Clinical Manager.
- Performs light housekeeping tasks related to creating a safe and comfortable environment, as directed by the nurse.
- Completes visit reports promptly and accurately within the electronic medical record system.
- Confirms daily visit schedules to ensure seamless coordination with other staff.
- Attends required in-service training sessions.
- Demonstrates awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participates as required.
- Understands and adheres to the agency’s process for managing and addressing customer complaints.
- Maintains acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
- Understands the agency’s policies regarding appropriate patient discharge and reasons for discharge.
