Position Overview: Occupational Therapist (OT)
As an Occupational Therapist at Pinnacle Home Care, you’ll play a vital role in delivering high-quality care to patients in the comfort of their homes. You’ll work under the direction of a physician and nursing supervisor, ensuring care meets all regulatory and clinical practice standards.
Key Responsibilities
- Perform initial and ongoing patient assessments, including OASIS documentation.
- Develop and implement tailored treatment plans in consultation with physicians, adjusting as needed.
- Collaborate with the interdisciplinary care team to optimize patient outcomes.
- Provide education and support to patients and families on therapy programs.
- Supervise Occupational Therapist Assistants every 30 days.
- Communicate patient progress and changes with physicians and care teams.
- Document all visits and progress notes on the same day of service.
Qualifications
- Graduation from an accredited Occupational Therapy program.
- Active OT license in Florida.
- Current BLS certification.
- Valid driver’s license and reliable, insured vehicle.
- At least one year of OT experience (home health experience preferred).
- Proficiency with electronic medical records is a plus.
- Physical capability to perform required tasks (standing, bending, lifting).
- Strong self-management, communication, and judgment skills.
