POSITION SUMMARY:
The Medical Social Worker (MSW) provides medical social services to clients in their homes based on physician orders and under the supervision of the Director of Clinical Services (DCS) or Clinical Manager (CM).
RESPONSIBILITIES:
- Assist with client admission by conducting an initial evaluation, assessing psychosocial status, and evaluating socioeconomic and emotional factors that may impact the treatment plan.
- Contribute to the development and implementation of the interdisciplinary care plan regarding medical social work.
- Monitor and report changes in the client’s emotional and social factors affecting their illness, care needs, and response to treatment.
- Collaborate with attending physicians to adjust the treatment plan as necessary.
- Maintain and submit clinical records, including initial evaluations, care plans, and daily notes, as required by the Agency.
- Evaluate the client and family’s response to medical social work interventions.
- Confirm visit schedules weekly with the Director of Clinical Services (DCS) to ensure coordination with other staff.
- Participate in agency activities and committees, including staff development and in-service education.
- Contribute to discharge planning.
- Pursue professional development through education, workshops, conferences, and research.
- Assess the client’s ability to cope with social and health challenges.
- Serve as a consultant to the healthcare team, providing insights into the social, emotional, and environmental factors influencing the client’s health.
- Assist clients in utilizing family and community resources.
- Provide rehabilitative and supportive casework to help clients and their families overcome personal and environmental challenges.
QUALIFICATIONS:
- Master’s degree in social work from a Council of Social Work Education (CSWE) accredited school.
- Licensed Clinical Social Worker (LCSW) in the State of Florida.
- Current BLS Certification.
- Valid driver’s license and access to a well-maintained, insured vehicle.
- Minimum of two years of experience, with at least one year in a healthcare setting (hospital, clinic, rehab center, etc.).
- Ability to meet the physical demands of the position.
