What are the responsibilities and job description for the Wound Care Coordinator position at Pinnacle Care Providers, LLC?
Job Purpose: Work alongside physicians and advanced practice nurses to ensure accurate and timely wound care coordination with high quality outcomes.
Responsibilities:
- Facilitate communication between physicians, advanced practice nurses, nursing facilities, patients, and patients' families.
- Coordinate and organize wound care census.
- Provide pre-rounding coordination of pertinent wound care data for physicians and advanced practice nurses.
- Duties will be performed in a facility where a patient resides (SNF, NH, AL, IL).
- Scribing patient encounters on behalf of physicians or advanced practice nurses as directed.
- Record and document physician and nurse practitioner visits on a patient roster to ensure consistent and quality care.
- Responsible for sending progress notes to the provider in an orderly and organized manner.
- Accurately bill patient visits in a timely manner.
- Must be willing to travel between facilities in Stark/Summit County area.
- Knowledge of Excel and experience in EHR.
- Excellent organization and time management skills.
- Ability to think critically.
- Team player.
- Thorough documentation skills.
- Conduct him/herself professionally.
- High comfort level working independently.
- Willing to listen and adjust behaviors based on physician/nurse practitioner feedback.
