About the Role
The Patient Navigator plays an integral role in supporting patients on their journey to parenthood. Working closely with a multidisciplinary team, you'll guide patients through their care plans, ensuring they receive the support and resources they need every step of the way.
We are seeking a Patient Navigator to join our team at California Fertility Partners , located in Los Angeles, CA . This is a full-time, onsite position , working 8 : 00 AM to 5 : 00 PM , with required holiday and weekend rotations, as well as additional on-call shifts .
Key Responsibilities
- Serve as the primary contact for patients, guiding them through treatment plans and educating them on procedures, testing, and follow-up care.
- Coordinate and schedule patient appointments, tests, and referrals to ensure seamless care.
- Assist patients in understanding their medical care, insurance coverage, and available financial resources.
- Collaborate with healthcare providers, clinical staff, and administrative teams to ensure continuity of care.
- Maintain accurate and up-to-date patient records in the electronic medical record (EMR) system.
- Provide emotional support to patients and families, addressing concerns and barriers to care.
- Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
- Educate patients on treatment protocols, self-care strategies, and medication usage.
- Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
- Advocate for patients' needs and assist in resolving barriers to care.
- Perform other duties and projects as assigned.
Position Requirements
Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
