Your Impact as a CSW
As a Community Support Worker in Raton, you'll be at the heart of our mission, directly touching lives and fostering positive change in our tight-knit community. Your role will be dynamic, rewarding, and never routine!
Key Responsibilities:
- Guide local clients toward their goals through personalized support and skill development
- Manage a diverse caseload of up to 25 clients within the Raton area
- Conduct meaningful home visits and provide crisis support
- Connect clients with vital community resources in Raton and surrounding areas
- Coach essential life skills for greater independence
- Support medication management and treatment adherence
- Empower clients in their decision-making journey
- Collaborate with treatment teams and families
- Document progress and maintain detailed records
- Participate in quality improvement initiatives
What You'll Need to Succeed:
- Raton residency (required)
- A passion for helping others and making a real difference in our community
- Strong communication and interpersonal skills
- Reliable transportation and valid insurance
- Proficiency with technology (Microsoft Office, EHR systems (EMR Bear), telemedicine platforms)
- Professional boundary-setting abilities:
- High school diploma/GED

Pay: $17.00 - $20.00 per hour
