      PIONEER CENTER for Human Services 
QIDP (Qualified Intellectual Disabilities Professional) Residential

Department: IDD Residential      Job Status: Full Time  

FLSA Status: Non-Exempt      Reports To:  IDD Residential Manager of Administration

    Amount of Travel Required: 40-50%

POSITION SUMMARY
The focus of the Residential QIDP/case manager is to support adults with Intellectual Disabilities that live within Pioneer’s 24-hour CILA group homes. The QIDP serves as the primary advocate for each client on their caseload, ensuring not only compliance, but quality of life and general happiness within all programs. 
In addition, the QIDP is responsible for the oversight and accuracy of nightly attendance and submission to billing for reimbursement monthly.

Essential Functions Statement(s)
    Maintain regular, reliable, and predictable attendance.
    Must possess excellent verbal and written communication skills.
    Meet deadlines for compliance expectations set forth in agency and outside agency. 
    Follows HR (Human Resources) policies, completes HR paperwork, and contacts HR with questions as needed.
    Ensures HIPPA compliance in all communication. 
    Serves as primary point of contact for guardians and/or families. Facilitates communication between guardians and/or families, and relevant agency staff.
    Serves as primary point of contact and makes decisions during crisis situations involving any client in residential. 
    Manage caseload of individuals with intellectual and developmental disabilities receiving residential services.
    Primary client advocate for all clients on caseload, ensuring quality care in all programs, medical offices, and in the community.
    Collaborates with the interdisciplinary team (guardian, client, Service Inc, QIDPs, outside service providers) to develop and implement the following (may include but not limited to)
o    Discovery Meeting, Person Centered Plan, Implementation Plan, discharge meeting, admission meeting, and any IDT meeting to support clients.  
    Responsible for completing monthly/yearly documentation including the Implementation Strategies, Service Reviews, assessments, consents, Critical Incident Reports, and progress notes/billing. All required information is entered into SmartCare (Agency’s Electronic Health Record System), where and when appropriate.
    Follow the SMART module when writing goals by keeping them specific, measurable, achievable, realistic, and time oriented. 
    Maintains accountability for initial and ongoing training of the Implementation Strategies and goal data tracking sheets.
    Must prepare all relevant paperwork and present it to the Human Rights Committee (HRC), and Behavior Management Committee (BMC), as needed.
    Provides oversight in medical management needs for over health and concerns.
o    Examples: Tardive Dyskinesia screening, medication review/management, annual psychiatric evaluations. 
    Assist in psych and neuro appointments as necessary for increased behavioral or dementia related concerns.
    Ensures compliance with requirements legally mandated by the Illinois DHS (Department of Human Services) related to program that participants are served in. 
    Ensures compliance with auditors such as BQM (Bureau of Quality Management), BALC (Bureau of Accreditation, Licensure, and Certification), CARF (Commission on Accreditation of Rehabilitation Facilities), HCBS (HOME AND COMMUNITY BASED SERVICES) Rule, etc. Assist in audits, as necessary. 
    Supports client in providing the opportunity for integration and access to the community, choice, individual rights, an independence in accordance with the HCBS Rules 
    Ensure accuracy of nightly attendance and submission to billing for reimbursement monthly. 
    Must be knowledgeable about and proficient in the application of all DHS requirements with which the agency must comply.
    Participants in an On-Call rotation for crisis calls/hospital visits, as necessary.
    Conducts at least biweekly visit with each assigned client
    Conducts check-in visits with clients while in hospital and in rehab and remains an advocate for care. 
    Communicates staff performance concerns with assigned House Supervisor. 
    Performs all other case management duties as assigned by Supervisor.


TRAININGS 
    Computer Systems: SmartCare (Electronic Health Record System), Teams (Communication platform), Policy Tech (Compliance documents/forms), Relias (Agency Training)
    Direct Service Professional Training. (Required before becoming a QIDP)
    QIDP certification
    DHS training on Rule 115 and 116
    Assessments- SLOF, ICAP, Safety assessments, Satisfaction Survey, etc. 
    CIRAS (Critical Incident Reporting and Analysis System) reporting and ICAP redeterminations. 
    In-house agency training such as Safety Care, CPR, OIG, Rights, Hazardous Chemicals, etc.

SKILLS & ABILITIES

Education:    Bachelor's Degree (four-year college or technical school) Required, Field of Study: Human Service can include but not limited to sociology, special education, psychology, criminal justice.  Proof of education must be provided. 

Experience:    Must satisfy all requirements to be recognized by DHS as a QIDP, including 1 plus year of experience working directly with individuals with a developmental disability.  One (1) plus year of experience in QIDP role preferred. Proof of experience must be provided.

Certifications &
Licenses:     QIDP certification
 

Other Requirements:    Must have and maintain a valid driver's license. 

Maintains 12 CEUs annually as required to uphold QIDP certification once obtained.


Must be able to meet agency driver safety requirement of carrying personal vehicle insurance with the following limits:
$25,000 bodily injury, each person
$50,000 bodily injury, each accident
$25,000 property damage

Ability to use Phone, Fax, Copier, and file required.
