Position Summary:
The Central Admission Coordinator manages the facilities census by developing relationships and maintaining positive relationships with hospitals, physicians, insurers, guardians and community groups.
Responsibilities:
Monitors, evaluates and responds to referrals for resident placement in collaboration with liaisons, Administrator and DON
Effectively manage occupancy levels in accordance with your budget
Act as a liaison between the facilities and the community
Responds to inquiry calls from hospital discharge planners, families and all other referral sources
Manages the admission process by maintaining updated bed availability and facility services information at all times
Assures that any DME needs are addressed prior to resident’s arrival
Communicates admission information to IDT members
Initiates the patient/resident medical record
Upload all necessary admission documents into the medical record
Reviews admission contract and required notices with residents, answers questions and obtains signatures within required timeframes
Assures that the patient/resident’s accommodations are ready prior to anticipated time of arrival
Verifies all insurance coverage prior to admissions
Completes daily and weekly reports and audits
Maintains confidentiality standards with all patient information and medical records
Notifies the billing office of all discharges and provides a face sheet and documentation required to support billing
Other duties as requested and assigned
Minimum Qualifications:
High school diploma or GED required. Associates or Bachelor’s Degree in Business or related field preferred
1 year experience in a long-term care setting in admissions, utilization review and/or accounts receivable strongly preferred
Computer skills at the level to accomplish job requirements
Strong organizational skills
Ability to relate professionally to all customers
Must be able to work independently, self-motivated and goal oriented
Ability to effectively communicate both orally and written form
Must be proficient in MatrixCare
