Job Title: Patient Care Coordinator - TOC
Supervisor: Supervisor PCCs/TCM-CCs
Director – Population Health
Qualifications:
1. Must be a Certified Medical Assistant/Licensed Practical Nurse/Registered Nurse
2. Previous experience preferred, but not required.
3. Must be able to perform any clinical or clerical duty as assigned skillfully.
4. Must comply with all OSHA and HIPAA regulations.
5. Must demonstrate strong verbal and communication skills.
6. Must demonstrate the ability to handle stressful situations appropriately.
7. Must be able to work flexible hours as needed.
8. Must demonstrate ability to work as part of the team.
9. Must maintain patient confidentiality.
General Duties:
General duties include, but are not limited to, the following abilities:
1. Participate in care of Paradigm Senior Health Connect patients with a focus on high risk and chronically ill patients recently discharged from a hospital or post-acute setting.
2. Initiate and follow through on home health care, palliative care, renal care, and hospice referrals.
3. Complete weekly outreach calls 30 Day TOC patients.
4. Collaborate with Transitional Care Nurse, Care Manager, providers and office staff on new program initiatives focusing on high-risk patients.
5. Carry out occasional home visits.
6. Perform other duties as assigned.
Location: Rotation between Administrative office in Green, Ohio and varying practice sites.
Physical Demands
1. Requires sitting for extended periods of time.
2. Some bending stretching, lifting, and stooping may be required.
3. Requires manual dexterity sufficient to operate office equipment including but not limited to telephone, keyboard, copier and fax machine.
4. Requires normal range of hearing and vision.
5. Requires the ability to communicate with patients, office personnel and the community.
